Case based discussions

Elise: managed to generate 30 minutes of discussion based on a normal elbow x-ray

Jolmer: expert of the mallet finger

Elisa: Gartland 3 supracondylar fracture

Tom: a retrieval case full of non-technical factors

Right ventricular resuscitation

  1. Don’t give fluids! (except perhaps frusemide)

  2. Ventilate sensibly (not too much PEEP, not too much tidal volume, not too much CO2)

  3. Increase coronary perfusion pressure with noradrenaline or vasopressin

  4. Add milrinone if available

Ultrasound

We can out of time to practice some RV assessment. This will be incorporated into another teaching session before the end of the year.

Simulation

A couple of mini sims:

  • MET call on the ward for rapid Afib and hypotension needing DCR

  • hypotension in septic patient with unrecognised accidental disconnection of vasoactive infusion
