referral pathways
MEDICAL ADMISSIONS
There are three medical inpatient teams: SMP1, SMP2, and Physicians. Deciding which team the patient should be admitted under can be challenging.
All children are admitted under SMP2.
Was the patient admitted to hospital less than three months ago?
If yes, call the doctor for the team they were admitted under last time (through switch)
Look at the sign next to the ED coordinator’s desk to identify which is the admitting team for the day.
Through switch, call the doctor belonging to the team on call
SURGICAL ADMISSIONS
Call the surgical on call doctor. The surgical consultant needs to know about all surgical admissions occurring overnight.
SHORT STAY UNIT
Discuss with the in-charge consultant and do the paperwork (found on the right hand side of the coordinator’s desk). Continue to provide care if you will be discharging them on your shift. If not, copy and paste your notes on WebPas into the NACS short stay discharge summary (once the have been “admitted” on the system).
ORTHO
Fracture Clinic appointments should be made for patients who actually have casts on.
Any other “? fracture” or other fracture clinic type patients who don’t have casts should be advised to seek GP review.
HANDS
Ed Yeboah has asked that we simply contact him directly if we have a hand referral, rather than using the “diary” process as done previously. It would be reasonable to wait until morning to contact him for those of you working overnight, unless absolutely necessary.
